    Procedure to open accounts in the MP 63 Fund

    United States Citizens and Residents


    Click on Forms at the Fund Website, MP63Fund.com or DRIPX.com.
     

    1. 1. Print out a Fund Application Form. Fill out the form and return it to the address below with a check for any amount (the MP 63 Fund does not require a minimum investment amount).

       
    2. 2. Provide copy of a US Government ID (like a driver's license).

       
    3. 3. If the document provided doesn’t have your address, include proof of your address, like a local utility bill.

      1. 4. Alternatively, you may wire transfers to fund your account: Your initial investment can be made by a wire transfer from your bank.

         
        1. A. First, call Mutual Shareholder Services (MSS) at 877-676-3386 to get your MP 63 Fund Account Number and to alert them that a wire will be coming from your bank. (If no one is available, dial “O” and explain that you are sending a wire to establish an account with the MP 63 Fund. You will be assigned an Account Number.)

        3. B. Then instruct your bank to send the wire to the following address:
