THE MP63 FUND

Procedure to open accounts in the MP 63 Fund

Foreigners who live in foreign countries

Go to the Fund Website, MP63Fund.com or DRIPX click on forms
 

1. 1. Print out a W-8BEN Form and a Fund Application Form.

    

2. 2. Enclose a copy of your passport or your local government issued ID (like a driver’s license).

    

3. 3. If those documents don’t have your address, include proof of your address, like a local utility bill.

 

4. 4. If your initial investment is more than $3,000, also provide proof of funds, which could be a bank account statement. (the MP 63 Fund does not require a minimum investment amount).

 

5. 1. 5. To fund your account: get a check or bank draft in US dollars and mail it the address below with the documents described above (W-8BEN Form, Fund Application Form, proof of address, and possibly proof of funds).

    

   1. 6. Alternatively, you may wire transfers to fund your account:

      1. A. First, call Tina Olexo, Mutual Shareholder Services (MSS) at 1-440-922-0066, ext. 102 to get your MP 63 Fund Account Number and to alert her that a wire will be coming from your bank. (If Tina is not available, dial “O” and explain that you are sending a wire to establish an account with the MP 63 Fund. You will be assigned an Account Number.

       

      2. B. Then instruct your bank to send the wire to the following address:

          

   U.S. Bank, Swift

   Cincinatti, OH

   For credit to the MP 63 Fund,

   Your Account Number ____________

   [which you were provided by Tina]
